This survey is being distributed by Plan4Health Swedesboro, a local initiative focused on improving community health through informed land use planning.  The initiative aims to bring community voices into decisions that affect health, land use, transportation, housing, the environment, access to healthy food, and access to healthcare.  All of these issues are interconnected and are so integrated into our daily lives that it is easy to overlook the factors that impact them.  Data and understanding are critical to our work.  We need input from residents of all ages!
